You can continue to roll after hitting the final level if you are seeking a defect. 
If you fail to roll a defect after spending 500 crowns you should automatically be given the the Charred Incarnation item with the trained character attatched. The design must be updated to have the defect visible before Defect Bonuses can be applied.
